Mending Life Coaching Agreement, Terms & Conditions
Our Goal: Our goal in providing Mending Coaching is to help you meet the challenges of life in a way that brings freedom of thought and healthy behavior to your life. We believe in a faithful worldview that puts Jesus as our example and savior. Mending Life LLC respects all people and prioritizes the individual journey over agendas or faith stories. As a recipient of coaching services, you don’t have to be pro-Jesus, just anti-chaos.
The function of a Mending Coach is to confidentially help individuals work on a healthy balance in life, provide a safe listening space, develop tools, overcome obstacles, and give support to decision-making and relationship building. Coaches assist with maintaining stability, growth in resilience, and mental and emotional health. Coaches function as helpers in recognizing where an individual is in life, what they don’t like about it, and establishing goals to get to a place they’d like to be.
Coaching Boundaries: The Mending Life LLC coaches are not trained or licensed as psychotherapists or mental health professionals, nor should they be expected to follow the methods of such specialists. Therefore, if you have significant legal, financial, medical, or other technical questions, you should seek advice from an independent professional. Our Mending Coaches will be happy to cooperate with such advisors and help you consider their counsel. Our Mending Coaching program is based on the training content found through AACC-certified Mental Health Coach training and various other trainings and education.
Fees and Payment: Coaching is offered in one-hour segments unless otherwise agreed upon. The fee for coaching is per hour and is subject to change with demand and service type, payable within 30 days. Mending Life LLC. Is a direct-bill only practice and is happy to provide letterhead invoices and proof of business and certification for the purpose of arranging HSA (Health Savings Account) payment forms.
Confidentiality: Confidentiality is an important aspect of the Mending Coaching process, and we will carefully guard the information you entrust to us. By signing this consent form, you give permission to your certified coach and their pastoral or clinical overseer the freedom to consult with one another if needed (we intend to create a supportive system and accountability for each participant). There are two situations when it may be necessary for us to share certain information with others: when a Mending Coach is uncertain how to address a particular issue and needs to seek advice from a professional and when there is a clear indication or possibility of harm to self-and/or others, elder abuse and/or child abuse, coaches are considered mandatory reporters in Oregon.
Termination of Services: Either party may terminate coaching services at any time with written notice. The Coach reserves the right to terminate the coaching relationship if participation guidelines, safety protocols, or presence requirements are violated.
Client Responsibility: As a participant in the Mending Coaching process through Mending Life LLC, you are responsible for your participation and growth in well-being, including decisions, choices, actions, and results of the coaching interactions. Please consider showing up on time and respectfully doing the work.
Resolution of Conflicts: On rare occasions, a conflict may develop between parties in a Mending Coaching situation. To make sure that any such conflicts will be resolved in a biblically faithful manner, we require all our Mending Coaches and participants in the Mending Coach program to agree that any disputes that arise with the above parties or with the sponsoring organization will be settled by biblical standards. For example, if an offense is too serious to overlook, or if we think someone may have something against us, we will go to each other to seek reconciliation, Matt. 5:23-24; 18:15. Or, if two of us cannot resolve a conflict privately, we will seek the assistance of trained people in mediation and listen humbly to their counsel, Matt. 18:16; Phil. 4:2-3. If we need further help, we will seek the insight of professional mediators.
Scheduling & CancellationScheduling & Cancellation: Sessions are scheduled by mutual agreement. Cancellations require at least 24 hours’ notice. Repeated cancellations or no-shows may result in termination of services and billable hours.
AI Note taking: Your clinician has opted to use Blueprint’s note-taking system as part of their effort to provide excellent care to clients. Blueprint’s note-taker temporarily records sessions and uses this recording to automatically generate a progress note (a required form of clinical documentation). Progress notes are automatically deleted from Blueprint’s servers.
Using this technology, your coach can be fully present during your sessions without slowing down to take notes to remember important information. This means they can focus all their attention on your care.
Blueprint’s software is HIPAA compliant and SOC 2 Type 2 certified, which means an external third-party auditor reviews Blueprint’s systems, policies, and processes annually to ensure Blueprint meets data privacy and security standards. By signing this consent form, you agree to allow your clinician to record your sessions and utilize software to assist them in generating progress notes to document these encounters.
Youth Eligibility & Consent: Services are offered to minors (under 18 years of age) only with the written and ongoing consent of a parent or legal guardian. By signing this agreement, the parent/legal guardian consents to their child receiving coaching services.
Parent or Guardian Presence Requirement For all coaching sessions:
- A parent or legal guardian must be physically present either in the same room or on the premises for the entire duration of each session.
- This ensures the safety and support of the minor client and promotes transparency throughout the coaching process.
Additional Requirement for opposite gender coach/client sessions
When a coach is assigned to work with a client of opposite gender, the following condition applies:
- A same gender (as client) pastor, staff member, or approved adult representative must be present in the room or within visual/audio supervision during all sessions.
- This provision is intended to ensure comfort, safety, and accountability for all parties.
